Files are rejected far more often because of a wrong certificate than because of bad insurance. Here is exactly which document the consulate and extranjería expect, and how to check yours before you submit.
A policy and a certificate are not the same thing
The policy is the contract, often dozens of pages. You do not submit it in full. What you need is a certificado de seguro — a separate one- or two-page document stating the essentials clearly and unambiguously.
The good news: insurers who work with foreigners issue this certificate as a PDF automatically, right after purchase.
What the certificate must show
Check it point by point — this is precisely where applications fail:
- Full name and passport number of the insured person, exactly as in the passport, letter for letter.
- Name and details of the insurance company, licensed in Spain.
- Validity — as a rule at least one continuous year.
- Full cover within Spain: primary care, specialists, hospitalisation and surgery, diagnostics, emergency care.
- An explicit statement: no co-payments (sin copagos) — why this matters.
- An explicit statement: no waiting periods (sin carencias) — what those are.
- No excesses or caps that would make the cover partial.
If the certificate does not spell out the absence of copagos and carencias, it will most likely be rejected — even if the policy behind it is a good one.
The usual reasons for rejection
- Submitting travel insurance instead of a health policy. It works for short Schengen stays but not for residency.
- A policy with co-payments — the classic “saved money, applied twice”.
- Validity shorter than required, or a start date later than the submission date.
- The name spelled differently from the passport.
- An insurer not licensed in Spain.
Language
The certificate is issued in Spanish — the working language of both consulates and extranjería. Some consulates accept a bilingual version. A sworn translation of a document from a Spanish insurer is normally unnecessary, but check your own consulate’s requirements in advance.
When to arrange it
The policy can be taken out before you move — the foreigner product lines are designed for it, typically several months ahead of your arrival date. The certificate arrives immediately, so you submit with the document already in hand.

If the visa is refused
With an official refusal the insurer normally refunds the premium against the refusal document. That is standard practice on foreigner product lines. In every other case (changed your mind, changed your plans) the policy is not refundable.
